Beth has had the police visit her on ward for attacking staff and has been cautioned. This is due to fighting back when in restraint and been on high alert. It's a vicious cycle- she can't have a discharge plan because she self harms and attacks staff and this has gone on for the 3 years. Prior to this, Beth was a grade A student. Excelled at school and college with her grades and was never in trouble.
Since been in a place that is overwhelming, loud, chaotic and doesn't offer structure or a routine she started attacking staff when in crisis.
If this doesn't show that in almost 3 years that the behaviours still continue and that it is down to the environment and procedures then I don't know what does.
