I have exhausted all known options, including:

PALS, CQC, NAS, PDA society, local MPs, MIND. I have shared on local pages. I didn't want to do this but I am out of options.

I am not invited to meetings (staff can't understand why that hasn't happened) to speak to care co ordinator or social worker (who is in Lincoln and we are in the North East) I've not had any communication or met them.

Beth has no idea of what is happening as she has no paper copies of anything and the hospitals don't know either! Even if she was told verbally, she can not retain lengthy conversations and when asked, she doesn't know.

Her care coordinator and social worker should be looking at possible accommodation to meet need, whether that's home, supported living or residential but Beth or her family have no idea what is going on.

Beth has an advocate but when I mentioned a mental health solicitor to make sure that things are actioned and happen, I was told that's what the advocate does. Yet, I raise the question.....

why didn't Beth have a copy of her care plan and support plan, why has she not got any paperwork of outcomes of meetings that she didn't attend due to anxiety, why aren't SALT, OT, Autism Team working alongside to help Beth manage her communication in a more appropriate way instead of ligaturing to gain the attention from staff, why hasn't she had exposure therapy and supported with life skills to get used to social situations and to be able to manage outside of hospital.

Beth won't ask these questions as it's processing the information and translating it to her advocate so I am her voice at meetings.

The recent meeting it was said "there is no discharge plan and that PUCU is the present to get Beth settled and another meeting will discuss next steps"

As soon as you are admitted to hospital, the people responsible for your healthcare should start assessing your needs and planning your discharge. This is stated in the English Mental Health Act Code of Practice and guidance from the National Institute for Health and Care Excellence (NICE). (MIND)

2 year and 10 month to not even have moved to next steps to closer to being out of hospital into the right setting, with the right support.
